Home Companionship Services

Sometimes seniors find themselves unable to adequately care for themselves while still being healthy enough to be at home. They just need a little help. That’s where elderly or disabled residents can really benefit from the in-home care services that Clover Senior Care provides.

RN Case Management Services

Healthcare can be a major concern for seniors. Clover Senior Care can help oversee medical care and monitor the overall health of elders in our care. This service is especially helpful when seniors do not have close family members nearby. Some of the services performed by our RN case managers include:

  • Perform Head-to-Toe Assessment – We will evaluate the general health status of seniors including:
    • Vital signs
    • Nutritional status
    • Posture, gait, balance, and mobility
    • Cognition, mood, orientation, language, and memory
  • Manage Care Plan – We will formulate and oversee a plan to promote continued health. This may include attending doctor visits, acting as a liaison with healthcare providers, and performing wellness checks.
  • Advocate for Patient Welfare – We will use our knowledge of medical terminology and disease processes to interact with other healthcare professions and represent the best interests of patients. In addition, we provide explanations to help our clients and their families better understand medications, diseases, and medical procedures.
  • Medication Organization and Education – We can help our clients to implement an organized system for any prescribed and/or over-the-counter medication.

Home Transition Services

Moving can be difficult, time-consuming, and stressful. There are just so many details to take care of. Relocation and downsizing for seniors, whether to a nursing facility or an assisted living facility, often involves additional effort. Clover Senior Care understands the unique challenges facing seniors who are moving, and we can help relieve some of the stress of moving with our Senior Home Transition Services. Some of the typical services we can provide for senior relocation include:

  • Initial evaluation to determine what services are needed.
  • Help finalize paperwork, such as signing of lease and obtaining medical documentation.
  • Assist with the physical move including:
    • Packing, decluttering home, and selling belongings as needed. We can also connect clients with a licensed realtor to list the home for sale.
    • Unpacking, organizing, and setting up new home. (We try to keep the set-up of the new residence as similar to the old set-up as possible as this is often helpful to a smooth transition.)
    • Canceling or transferring utilities and other services, such as gas, electric, phone, internet, cable, and newspaper delivery.
  • Coordinate any required legal services, such as to create a living will or appoint a power of attorney.
  • Assist in end of life preparation.
