PHYSICAL THERAPIST ASSISTANT
The Physical Therapy Assistant, under the direction of a Registered Physical Therapist and
Director of Nursing, performs patient care as directed by way of a written plan of care and
in accordance with state practice guidelines.
Requirements:
●Current Physical Therapy Assistant License in the state of Indiana.
●Valid drivers license, valid auto insurance, and reliable transportation.
●Current CPR/In-Person certification.
●Valid TB test (past 12 months) or have one placed in our office.
●Tech savvy, able to learn and navigate several platforms. Proficient with Apple
products.
●Perform routine duties independent of supervision but consults with others as
required.
●Excellent communication and listening skills.
Preferred:
●Proficient in using EMR systems, (Axxess).
●Minimum of 2 years Home Health experience.
●General knowledge of home health practices, including familiarity with relevant
state and local services.
Responsibilities:
●Evaluates abnormal conditions by reviewing physical therapists’ interpretations of
patient evaluations and test results, send/upload progress notes/documentation for
PT review.
●Document patient care services by charting in patient and department records.
●Performs therapeutic procedures by administering manual exercises and
instructing, encouraging, and assisting patients in performing physical activities,
such as non-manual exercises, ambulatory functional activities, and daily living
activities.
●Conducts weekly patient visits with ongoing visits as ordered by the Physical
Therapist.
●Promotes and maintains health by providing physical therapy assistant services
under the supervision of a physical therapist.
●Contributes to a physical therapist’s effectiveness, identifies patient care issues.
●Accesses patient health by interviewing patients, performing physical examinations
and obtaining, updating and studying therapy histories.
●Maintains a safe and clean working environment by complying with procedures,
rules, and regulations, and adhering to infection-control policies and protocols.
●Maintains production objectives by noting current treatment trends and monitoring
actual physical therapy services rendered to production objectives.
●Maintains legal and accreditation compliance by adhering to policy positions
concerning federal, state, and local regulations and JACHO standards.
●Updates job knowledge by participating in educational opportunities, reading
periodicals, maintaining personal networks. Enhances physical therapy reputation
by accepting ownership for accomplishing new and different requests and exploring
opportunities to add value to job accomplishments.
