Job Details:

The Registered Nurse is responsible for the delivery of patient care through nursing processes. You will provide in-home direct patient care according to the provider’s prescribed plan of care (and nursing scope of practice) while closely monitoring progress/recovery of the client. Work in accordance with agency philosophy, standards of professional nursing practices, and all federal and state laws and regulations, including HIPAA as applicable.

Expectations:

● Complete skilled assessments and provide exceptional patient-centered care in the home.

● Use clinical tools, data and other information to complete assessment of client needs.

● Document complex medical information to outline and improve health results that help determine immediate and routine care for the client.

● Educate clients and families on benefits, services and other resources available to them.

● Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve.

● Demonstrate knowledge in medical care; such as diagnostic procedures, medication, symptoms, and other treatment-related therapies.

● Work to decrease re admissions by promoting preventative care and ensuring continuity of care.

Qualifications:

● Associates or Bachelors degree in Nursing from an accredited nursing school.

● Active nursing license in the state of Indiana.

● Current CPR/First Aid certification, Current TB screening test, pass a national background check.

● Strong written and verbal communication skills.

● General knowledge of case management practices, including familiarity with relevant products and services.

● Valid Driver’s License and Automobile Insurance.

● Home Health Care experience preferred. NEW GRADS WELCOME!

$25.00 – $55.00 per hour
