Job Overview
The Speech Therapist (Speech-Language Pathologist) assesses, diagnoses, and treats speech, language, communication, and swallowing disorders to help patients improve independence and overall quality of life. Working under the guidance of the Director of Nursing, this role provides speech therapy services in the home setting, develops individualized care plans, and delivers evidence-based therapeutic interventions. The Speech Therapist collaborates with patients, families, and healthcare professionals to support recovery, promote functional communication, and help patients regain daily living skills.

 

Responsibilities:

Assessment and Evaluation

  • Conduct Initial Assessments: Evaluate the patient's speech, language, and swallowing abilities to identify specific needs.

  • Create Treatment Plans: Develop individualized therapy plans based on assessment results and patient goals.

Therapy and Intervention

  • Provide Speech Therapy: Implement exercises and activities to improve articulation, fluency, and voice quality.

  • Facilitate Language Development: Work on enhancing understanding and use of language, which might include vocabulary building and sentence structure.

  • Swallowing Therapy: Assess and treat swallowing disorders, providing strategies to ensure safe and effective swallowing.

Education and Support

  • Patient and Family Education: Teach patients and their families about communication strategies and swallowing techniques.

  • Support Caregivers: Provide guidance on how to assist patients with their communication needs in daily activities.

Documentation and Reporting

  • Maintain Records: Document patient progress, treatment sessions, and any changes in the patient's condition.

  • Collaborate with Other Healthcare Providers: Communicate with physicians, nurses, and other therapists to ensure coordinated care.

Follow-Up and Adjustments

  • Monitor Progress: Regularly evaluate the effectiveness of therapy and adjust treatment plans as necessary.

  • Set Goals: Help patients set realistic and achievable goals for their communication or swallowing abilities.

 

Requirements:

  • Masters in Speech Therapy or Speech Language Pathology from an accredited college or university

  • Active Indiana Speech Therapy license

  • 2+ years experience working as a Speech Therapist or Speech Language Pathologist

  • Ability to drive up to 30 minutes minimum for scheduled client shifts

  • In-person CPR certification via the American Red Cross or Heart Association (*can be provided on-site if needed, at cost)

  • TB test within the past 12 months (*can be provided on-site if needed, at cost)

  • Valid driver’s license, reliable transportation, vehicle insurance, and vehicle registration

  • Excellent communication, organization, and time management skills

  • Ability to work independently while collaborating with the care team

 

Preferred Skills and Experience:

  • Home Health experience

  • OASIS documentation experience

  • Experience using AXXESS EMR

  • Strong technology skills; proficiency with Apple devices and applications

 

Schedule:

  • Monday-Friday (Weekend availability)

  • Part-Time: 16-29 visits per week

  • Full-Time: 30-35 visits per week
