Job Overview
This role offers an exciting opportunity to deliver personalized nursing care in patients' homes, ensuring optimal health outcomes through comprehensive assessments, plan of care, and patient education. The ideal candidate will possess a strong clinical background, excellent communication and documentation skills, and a passion for community-based healthcare.

Responsibilities:  

Patient Assessment

  • Conduct Comprehensive Assessments: Evaluate patients' physical and mental health, including vital signs and medical history and living condition/environment.

  • Monitor Progress: Regularly check on the patient’s condition and update care plans accordingly.

Care Planning

  • Develop Individualized Care Plans: Create tailored plans based on assessments and patient needs.

  • Coordinate with Healthcare Teams: Collaborate with physicians, therapists, and social workers to ensure comprehensive care.

Patient Education

  • Teach Self-Care Techniques: Instruct patients and families on managing conditions, medication administration, and lifestyle changes.

  • Provide Emotional Support: Offer counseling and support to patients and their families regarding health issues.

Medication Management

  • Administer Medications: Safely give medications as prescribed and educate patients on their use.

  • Medication setups, education, and supervision.

  • Monitor Side Effects: Keep an eye out for any adverse reactions or side effects from medications and teach side effects.

Wound Care and Treatments

  • Perform Wound Dressings: Manage and care for wounds, including changing dressings and cleaning.

  • Administer Treatments: Provide necessary treatments such as IV therapy or catheter care.

Documentation

  • Maintain Accurate Records: Document all assessments, interventions, and patient responses in medical records.

  • Report Changes: Communicate any significant changes in the patient’s condition to the healthcare team.

Safety and Compliance

  • Ensure Safety Protocols: Follow infection control and safety protocols to protect patients and themselves.

  • Stay Compliant with Regulations: Adhere to state and federal regulations governing home health care.

Advocacy

  • Advocate for Patients: Represent the patient’s needs and preferences in care discussions and decisions.

  • Facilitate Resources: Connect patients with community resources and services as needed.

Requirements:

  • Associate or Bachelor’s degree in Nursing

  • Active Indiana RN license

  • 2+ years nursing experience

  • Ability to drive up to 30 minutes minimum for scheduled client shifts

  • In-person CPR certification via the American Red Cross or Heart Association (*can be provided on-site if needed, at cost)

  • TB test within the past 12 months (*can be provided on-site if needed, at cost)

  • Valid driver’s license, reliable transportation, vehicle insurance, and vehicle registration

  • Excellent communication, organization, documentationand time management skills

Preferred Skills and Experience:

  • Bachelor’s degree

  • Home Health experience

  • OASIS documentation experience

  • Experience using AXXESS EMR

  • Strong technology skills; proficiency with Apple devices and applications

Schedule:

  • Monday-Friday (Weekend availability)

  • Part-Time: 16 - 29 visits per week

  • Full-Time: 30 - 35 visits per week
