Job Overview
The Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient’s progress toward achieving functional goals. The Physical Therapist will also maintain a

 

Responsibilities:

  • Evaluation & Treatment: Conducting initial and ongoing assessments of patient functional status, safety, and physical therapy needs within their home.

  • Plan of Care: Developing and updating patient-specific plans of care in coordination with physicians, including interventions for pain management and increased independence.

  • Documentation: Submitting timely progress notes, evaluation reports, and discharge summaries, often using electronic systems.

  • Collaboration: Working with multidisciplinary teams, including nurses, occupational therapists, and home health aides.

  • Patient/Caregiver Education: Instructing patients and families on exercises, safety techniques, and home modifications.

  • Supervision: Overseeing the care provided by Physical Therapist Assistants (PTAs) and home health aides.

 

Requirements:

  • Masters or Doctorate Degree in Physical Therapy from an accredited college or university

  • Current Physical Therapy License in the state of Indiana

  • 2+ years experience working as an Occupational Therapist

  • Ability to drive up to 30 minutes minimum for scheduled client shifts

  • In-person CPR certification via the American Red Cross or Heart Association (*can be provided on-site if needed, at cost)

  • TB test within the past 12 months (*can be provided on-site if needed, at cost)

  • Valid driver’s license, reliable transportation, vehicle insurance, and registration

  • Perform routine duties independent of supervision but consults with others as required

  • Excellent communication, organization, and time management skills

  • Ability to work independently while collaborating with the care team

 

Preferred Skills and Experience:

  • Home Health experience

  • OASIS documentation experience

  • Experience using AXXESS EMR

  • Strong technology skills; proficiency with Apple devices and applications

 

Schedule:

  • Monday-Friday (Weekend availability)

  • Part-Time: 16-29 visits per week

  • Full-Time: 30-35 visits per week
