Job Overview
The Occupational Therapist (OT), under the supervision of the Director of Nursing, performs patient care as directed by way of a written plan of care and in accordance with state practice guidelines. OTs treat disabled, ill, or injured patients with special equipment and the therapeutic use of daily activities. They help patients improve, regain, and develop the skills needed for day to day life and work. They may provide long-term patient care and acute patient care. OTs will work with patients who are recovering from injuries and disabilities and help them regain their abilities so that they can resume work.
Responsibilities:
Comprehensive Assessment: Perform initial and ongoing evaluations of patients' cognitive, physical, and psychosocial abilities in their home environments.
Treatment Planning: Develop and implement customized treatment plans with measurable goals to improve functional performance.
ADL Training: Provide skilled therapy to improve Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs), such as grooming, dressing, and cooking.
Home Safety Assessment: Evaluate the home for safety hazards and recommend adaptive equipment or environmental changes to prevent injuries.
Education and Training: Instruct patients and family members on rehabilitation techniques, safety protocols, and the use of adaptive devices.
Documentation: Maintain accurate, timely electronic medical records (EMR) for visits, progress notes, and discharge summaries, complying with Indiana regulations.
Case Coordination: Collaborate with nurses, physicians, and other team members to coordinate care.
Supervision: Supervise Occupational Therapy Assistants (COTAs) and Home Health Aides.
Requirements:
Masters or Doctorate Degree in Occupational Therapy from an accredited college or university.
Current Occupational Therapy License in the state of Indiana.
2+ years experience working as an Occupational Therapist
Ability to drive up to 30 minutes minimum for scheduled client shifts
In-person CPR certification via the American Red Cross or Heart Association (*can be provided on-site if needed, at cost)
TB test within the past 12 months (*can be provided on-site if needed, at cost)
Valid driver’s license, reliable transportation, vehicle insurance, and registration
Perform routine duties independent of supervision but consults with others as required.
Excellent communication, organization, and time management skills
Preferred Skills and Experience:
Home Health experience
OASIS documentation experience
Experience using AXXESS EMR
Strong technology skills; proficiency with Apple devices and applications
Schedule:
Monday-Friday (Weekend availability)
Part-Time: 16 - 29 visits per week
Full-Time: 30 - 35 visits per week
