Licensed Practical Nurse

Licensed practical nurses primarily provide routine, day-to-day nursing care. LPN job responsibilities may include helping their patients bathe, dress and eat; go to the bathroom; and get their daily exercise. They change bandages, dressings and catheters, and in some states they can administer medication or IVs. Most importantly, LPNs monitor and assess their patients’ condition everyday. They often have more contact with their patients than other caregivers, and are better able to detect subtle changes that indicate improvement or deterioration in their condition. Most practice in hospitals and extended-care facilities, but many work in home health care.

Responsibilities Include but are not limited to:

● Assists the registered nurse/therapist in assessment of patient’s condition and contributes information to aid in planning and evaluating care. Considers the physical, psychosocial and emotional needs of the patient.

● Attend compliance training and adhere to the organization standards of conduct, policies and procedures

● Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction

● Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide

● Maintain a confidential record of patient’s progress, symptoms, reactions; treatments performed, interventions and care provided. Completes documentation including visit assessment and verbal orders accurately and promptly on the laptop device or in the written documentation while in the patient’s home and within the agency and regulatory standards. Data syncs (transmits) information same day. This information is communicated to the primary service provider and appropriate interdisciplinary team members in a timely and thorough manner.

● Maintaining nursing knowledge base through in services, literature review, mandatory and competency programs

● Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of Clover Home Health Care towards patients, staff, peers, physicians and other departments within the organization.

● Provide individualized patient/caregiver teaching and appropriate counseling related to health prevention, health maintenance and disease process under the direction of the registered nurse

● Providing individualized skilled home health nursing care to patients under the supervision of a registered professional nurse.

 

Qualifications:

● Completion of a Practical Nursing Program approved by the State Board of Nursing. ● Excellent communication skills are necessary to interact with patients effectively,  family members, visitors and staff members.

● Basic computer skills necessary to input patient care orders and medical record documentation.

●  Minimum of 3 years nursing experience strongly preferred.

● Criminal Background Check Clearance

● Automotive Insurance

● In Person CPR Certification

● Driver’s License

● Licensed Practical Nurse

● TB test (within that last year)

●  Hepatitis

● Urine Drug Screen Clearance
